Job Description:
Responsibilities Provide & assist in direct patient care under
the supervision of the charge nurse (RN) as prescribed by the
physician & the treatment team. Provide medical management and
re-assessment of patients. Assist RN with environmental safety and
crisis management for the patient population. Supervise MHTs as
directed by RN. Document patient care, ensure that data is
accurate, & that it is included in the master treatment plan via
timely nursing care plan updates. Work with the treatment team to
provide a consistently therapeutic milieu. Rockford Center is a
138-bed mental health facility located in Newark, DE, providing
easy access to a full complement of inpatient and partial
behavioral health programs for children and adolescents, adults,
and older adults. Rockford is the only child inpatient behavioral
health facility in DE, offers an experienced and tenured senior
management team, and serves New Castle County and northern
Maryland. CUSTOMERS SERVED: Children (age 4-12), Adults (age
18-64), Adolescents (age 13-17), Geriatric (age 65+) Patients;
families; direct-care service providers; external funding sources;
service, accreditation, and licensing agencies; external case

patient medications as prescribed by physician Maintains
appropriate documentation for Quality Improvement monitors,
Infection Control reports, etc. Provides education to patients on
their prescribed medications, as well as families, and document
timely and accurately in the patient's chart Under the guidance of
a Registered Nurse, completes nursing assessments and document, as
appropriate Completes transcription of medication orders timely and
error free Helps identify patient problems, needs and strengths,
and suggesting modifications to the treatment plan Completes
Adverse Drug Reaction forms and forward to the Pharmacist Maintains
patient, program, department and related documentation Collects and
analyzes data regarding the patients physical, psychosocial,
environmental, self-care, educational and anticipated needs
throughout treatment Conducts and documents ongoing patient
evaluation tools Medication administration and management of
patient medications and physician orders Participates as a member
of interdisciplinary treatment team Provides appropriate nursing
intervention/documentation in crisis situations Reports unsafe
patient conditions and change in status Provides age-specific
communication, limit setting and interventions Prepares and adheres
to the facility patient discharge process Assists patients in
carrying out activities of daily living Participates in hospital
emergency codes necessary to the unit's and the hospital's safe
operation Report any incidents or suspected incidences of
sexual/physical abuse or other issues to community/state agencies,
and represent the facility in any legal proceedings Other Duties
and Responsibilities: Report all accidents immediately and provide
effective timely corrective action on all accidents Will adhere to
all safety policies and safe work practice Will adhere to all
hospital policies and procedures Perform other duties as assigned

Education/Training: Graduate of an accredited school of practical
nursing Experience: One year inpatient psychiatric experience with
adults, adolescents and/or children preferred.
Licensure/Certification: Current Delaware Licensed Practical Nurse
License Current Driver's License CPR Certification Knowledge and
Skills: Knowledge of appropriate nursing assessment, diagnoses, &
interventions with patients. Knowledge of milieu management.
Accurate administration of medication & treatments. Monitor &
record response & side-effects to medication & treatments. Ability
to safely implement Special Treatment Procedures. Ability to
develop therapeutic relationships with patients and families.
Ability to assist with educating patients & families in medication
& care management. Ability to write clearly, legibly, accurately, &
concisely in patient charts. Skill in maintaining appropriate
medical record documentation. Ability to direct subordinates in the
performance of their duties.
