Job Description:
A company is looking for a Compact LPN LVN Licensed Care Team
Member.
Key Responsibilities:
Triage physiological markers like blood pressure, weight, and blood
glucose
Perform monthly wellness calls with assigned patients and lead
collaborative wellness calls to define health goals
Manage patient messaging and alerts, escalate urgent care needs to
patient's physician as appropriate
Required Qualifications:
Unrestricted LPN/LVN license in a compact state, preferably
Illinois
1-3 years of clinical experience, patient management, or disease
management desired
Experience with Medicare patients and virtual visits/telephonic
care management
Interest in professional leadership growth and development
opportunities
Health Coach certification a plus
