Hospice RN Job Details: Pay Rate: $1750-$1971 gross per week Schedule: 7p-7a, 3 shifts a week Available Shift: Nights, 3 shifts per week Start Date: 8/26 Located In: Newark, DE Requirements: Qualified applicants MUST have at least 1 year of experience in a Medicare-certified home health agency, have a valid RN license and be willing to obtain DE licensure. As a Hospice RN, you'll have the opportunity to provide comprehensive care to patients in the comfort of their own homes, fostering independence and promoting optimal health outcomes. As their in-home clinician, you will conduct comprehensive assessments of patients' health status and develop individualized care plans in collaboration with their interdisciplinary team. In the Home Health setting, you will be a vital link between a patient and their family to resources across the care spectrum. This mobile role offers independence and differentiation in your day-to-day work, while also keeping you connected to the coordination of care with other healthcare providers like physicians, therapists, and social workers.
