Job Description:

offer! Christiana Hospital in Newark, DE, a part of Christiana Care
Health System is seeking an RN to work as a Diabetes Educator. In
this position, you will work in various units and learn valuable
skills. To anticipate, coordinate, communicate, and monitor the
patient plan of care (POC) in clinical acute inpatient areas
through an interdisciplinary team approach, according to American
Diabetes Association (ADA) guidelines and/or recommendations and
ChristianaCare diabetes disease management protocols. Coordinate
and provide diabetes education to patients in clinical acute
inpatient areas specific to the ADA guidelines and/or
recommendations. The nurse navigator is responsible for the goal
attainment process and is accountable for identifying and removing
barriers that will prevent the patient from reaching these goals
General Requirements: Bachelor of Science in Nursing (BSN)
required. Masters degree preferred Delaware State RN license or
multistate compact RN license Current BLS required. Specialty
certification in acute care nursing preferred. Three years of
nursing experience required Experience working with at-risk
populations is highly desirable. Certification Board for Diabetes
Care and Education (CBDCE) or Board Certified Advanced Diabetes
Management (BC-ADM) certification preferred (required within 3
years). Bachelor of Science in Nursing (BSN) required. Masters

Certification Board for Diabetes Care and Education (CBDCE) or

preferred (required within 3 years). Primarily shift for this
position will Day shift with Weekend and Holidays per the
guidelines of the department. Hours: This position is 40 hours per
week. Principal duties and responsibilities Develop action plans
upon admission and ensure individualized diabetes management goals
are met prior to discharge while facilitating a safe, timely and
efficient transition to the next level of care. Participate in
updating and maintaining acute care diabetes-related guidelines and
educational materials for ChristianaCare, ensuring compliance with
the ADA recommendations. Demonstrate knowledge and skills necessary
to provide care, education, and support appropriate to adult and
geriatric diabetes patients. Obtain and interpret information to
identify patient needs and provide the care needed. Conduct an
initial assessment of patient care needs upon admission. Develop
and individualize a plan of care (POC) based on patient assessment.
Coordinate with case management a plan identifying likely discharge
location category and discharge needs. Assess the learning needs of
patients/families and carry out teaching as needed related to
diabetes self-management. Provide the patient/family with
information regarding the POC (diagnosis, medications, treatment,
tests, diet, etc.). Review patient progress during inpatient
admission, including but not limited to individualized diabetes
management goals, labs, response to medications, consults, and
orders to assure they are completed in a timely manner. By joining

This pay rate/range represents ChristianaCare’s good faith and
reasonable estimate of compensation at the time of posting. The
actual salary within this range offered to a successful candidate
will depend on individual factors including without limitation
skills, relevant experience, and qualifications as they relate to
