Job Description:
ChristianaCare is seeking a dedicated Clinical Pharmacist to
join our Inpatient Pharmacy team and deliver optimal medication
management across our healthcare settings. This role is pivotal in
ensuring the safe and effective use of medications in conjunction
with our mission to provide advanced, specialty, and primary
healthcare services. Our Clinical Pharmacists are vital to our
team, collaborating directly with medical staff to tailor
pharmacotherapeutic regimens for individual patients. By joining
our team, you will be stepping into a supportive role that promotes
improved patient outcomes and continued education in a rewarding,
community-focused healthcare environment. Evaluate physician
medication orders for appropriateness of drug, dosage, potential
interactions, route of administration and stability. Monitor
patient responses to drug therapies and report any adverse
reactions or issues. Collaborate with the healthcare team to
develop and implement pharmaceutical plans that achieve desired
therapeutic outcomes. Provide information and counseling to
patients on drug therapies. Participate in clinical research and
review of evidence-based practices. Ensure compliance with all
health care regulations regarding medication management including
storage and handling. Educate staff and interns within the pharmacy
department on medication therapy management. Strong understanding
of pharmacotherapy and advanced knowledge of drug interactions and
potential side effects. Excellent communication and interpersonal
skills to interact effectively with patients and healthcare team.
Analytical skills to assess complex drug regimens. Attention to
detail and precision in order fulfillment and record keeping.

401k Current and unrestricted Pharmacist Licensure in the state of
Delaware or eligible for reciprocation. Board Certified
Pharmacotherapy Specialist (BCPS) or willingness to obtain
certification within 2 years of employment.
