Community Living Specialists are located in Wilmington, Newark, Claymont, Dover and Millsboro, Delaware. The Community Living Specialist is an integral member of the Community Building team that provides recovery training, outreach and support to adults transitioning in the community and enrolled with Assertive Community Treatment (ACT) or Intensive Case Management (ICM) Services. The Community Living Specialist brings their lived experience of mental health and/or sobriety to the team, providing evidence that Recovery is possible.In partnership with the neighbor, the Team Leaders, and the neighbor's Treatment Team the Community Living Specialist facilitates the path identified in achieving the Neighbor's stated goals as identified on the Collaborative Treatment Plan. The Community Living Specialist acts as an advocate for the needs of every neighbor and works in collaboration with ACT/ICM teams to support the day-to-day activities of the program's Recovery environment.Essential Job FunctionsMaintain and observe all HIPAA regulations that apply.Within 24 hours of moving into a Community Building location, the Community Living Specialist ensures the Welcome Packet, including consents, is understood and completed.The Community Living Specialist facilitates the creation of a Collaborative Treatment Plan with the neighbor and their treatment provider.Enters information into Avatar, RI International's Electronic Medical Record.Advocates with the neighbor to ensure collaboration with identified community supports i.e. family, friends, clinical home, housing supports, etc., to obtain collateral information and ongoing participation in the neighbor's recovery journey.In partnership with the neighbor, serves as an advocate with treatment and other service providers in order to achieve success toward goals identified in the Collaborative Treatment Plan.Identifies and educates neighbors on community resources available to them, including but not limited to social, educational, and employment activities.In partnership with the neighbor, Community Living Specialist will prompt and/or monitor completion of activities of daily living. Activities of daily living include bathing, dressing, meal preparation, laundry, cleaning, shopping. Depending on the strength of the neighbor, if ADLs cannot be successfully completed by the neighbor, the neighbor's treatment team is notified.Adheres to attendance and punctuality policy.Dresses in an appropriate manner with the nature of work performed.May drive personal or agency vehicle to conduct agency business, if authorized to do so.Other duties as assigned.Documentation Documents relevant and required information and services provided in the identified service record (electronic or otherwise) assuring continuity of care as a tool for communication with other team members.Provide timely, quality and accurate documentation in accordance with program standards, using correct spelling, proper grammar, and ensuring that the correct treatment episode and unit are selected before entering notes in Avatar.Safety/ Physical Environment Participates as a team member to provide a safe recovery environment. Utilizes infection control and other safety standards. Contributes to the cleanliness of the work place.Utilizes effective positive engagement and conflict resolution skills.Fosters personal and environmental wellness by taking care of self and maintaining environmental wellness by reporting infection and on the job injuries.Assesses emergency situations, notifies supervisor of actual or potential challenges, exercises judgment in stressful conditions and responds calmly and quickly. Continuing Education & CertificationsBy employment date and annually thereafter, TB test clearance.Successful completions of FBI fingerprint background clearance, Adult Abuse and Child Abuse Registry Checks.Service Letter and or Professional Reference requirements.First Aid Certification, as required.CPR Certification, as required.Completion of all company required trainingDriving Requirements (if an authorized or approved driver):Must possess a valid State Driving license and be at least 21 years of age if driving a company vehicle or personal vehicle on agency business.Proof of current auto insurance if using a personal vehicle on agency business.Must be able to meet all of the requirements of the Organization's Driver's Safety Policy.Must meet and remain in compliance with all driver eligibility requirementsWorking Conditions:Works in an office environment, community and/or behavioral health clinic and may sometimes travel to meet the business needs.Works a standard work week but may be required to work some evenings and weekends to ensure program activities are met.May be exposed to agitated individuals, risk mitigated by de-escalation training.Physical Requirements:The physical requirements described herein are representative of those which must be met by an employee to successfully perform the primary functions of this job. Works in a standard office environment, community and behavioral health clinical areas with some exposure to hazardous situations. Physical demands may include work close to the eyes and substantial motions of the wrist, hands and/or fingers. Occasional sitting, standing, walking, handling, finger dexterity, eye-hand-foot coordination, use of corrected vision. May be required to lift up to twenty (20) pounds.Reasonable accommodation may be made to enable individuals with disabilities, who are otherwise qualified, to perform the primary function.
