Our mission is to radically improve the health, happiness, and dignity of senior living residents. We are seeking a skilled Primary Care Clinician to join our team in Oxford, PA. The successful candidate will administer direct patient care services within the Post-Acute Care setting, conducting clinical interventional and episodic direct care visits, and providing ongoing evaluation and treatment of chronic, co-morbid illnesses. Key responsibilities include reviewing medications, orders, and diagnostic tests, coordinating care with collaborating physicians, and maintaining communication with families. The ideal candidate will possess a valid nurse practitioner or physician assistant license, a Master's Degree in Nursing or Physician Assistant Studies, and strong leadership, interpersonal, and problem-solving skills.
